When your PMP or doctor prescribes a certain drug, be sure to fnd out what you need to know before you leave the ofce or clinic.

• Ask what the common side efects are with the drug.

• Repeat back to the PMP or doctor what he or she tells you about what the drug is for and when and how to take the drug. Tat way you know you understood what the doctor told you.

• When you see the doctor, bring along bottles for all drugs you take. Be sure to include any you buy over the counter at the drug store (even those for weight loss, cold, cough or allergy). List vitamins, herbs or homemade remedies too.

• If you are afraid of a drug or afraid of becoming addicted to it, talk with your doctor.

• Call the prescribing doctor if you think you are having side efects or want to stop taking a drug.

Looking for a provider or hospital?

Anyone can search for providers and hospitals in English and Spanish. Go to anthem.com . Choose the “Find a Doctor” tool on the home page.

This online provider directory lets you search for doctors and facilities by plan type, name, specialty, gender or location.

You also can search our Mobile Provider Finder if you have a cellphone or mobile device with web service. To use it, just type

www.anthem.com/mobile into your cellphone’s or device’s web browser.

If you don’t have Internet access, call us at 1-866-408-6131 . A staff member will search the web for you using the criteria you provide. He or she will then read, mail or fax a hard copy of the results to you.
