Te Care Management program can help . Care managers, who are registered nurses or other health care professionals, are here to help when you have a serious medical condition or unexpected medical event.

You, your family and your doctors work with a care manager, who provides telephone support at a time that works for you and at no cost to you.

Tis program is private and voluntary. It gives you extra support when you need it most. To be part of it, you can self-refer or your caregiver can refer you.

Te care manager will work with you to help you manage your condition. He or she also can answer questions about your benefts and help you fgure out what support and care you need. To learn more or be part of the program, call us toll-free at 1-866-408-6131 (choose

Pick an easy date to remember

It’s hard to remember when to get yearly well visits for your child or yourself. To make it a bit easier, schedule the visit on an important date in your life. You might use a birthday, anniversary or holiday. Think of it this way: good health is the best gift you can give anyone.

Get to know your PMP

As a member, you’re assigned to a PMP. It’s important that you use the PMP you are assigned to as your personal doctor. Make a point of going to visit the PMP for care if you need it. It’s a good idea to schedule a visit or checkup even if you are well. If you see a doctor who is not your PMP without an OK from us, some services may not be covered, depending on the type of care you get. There are exceptions. You can always change your PMP at any time for any reason.

It’s usually best to keep the same doctor so he or she can get to know your health needs and history. So start by calling your PMP for a visit or checkup.

Teens

Exams 4 u to ace

Health exams are not just for young kids. Preteens, teens and college students all need checkups and the vaccines that are part of those annual exams. Since some vaccines that younger children get can wear off, older kids need booster shots (extra doses) to stay protected from diseases. There also are certain vaccines made just for preteens and teens. The PMP can tell you which vaccines are right for your older child. Call your teen’s PMP to schedule a yearly checkup for your teen. It’s his or her best way to ace life’s exams.
